چکیده

This study assessed two 85-year-old patients diagnosed with diastolic heart failure and persistent respiratory insufficiency characterized by severe obstructive ventilatory disorder and gas exchange alterations. The possibility that the respiratory impairment was consequent to primary pulmonary disease was excluded. Radiological signs of mild pulmonary edema had been observed in 1 of the patients during the 4 years preceding the first hospitalization. The respiratory findings were attributed to pulmonary lesions conditioned to the presence of chronic pulmonary edema. The incidence of the disease in very elderly patients may create an illness that can be called congestive pulmonary disease.

Congestion in acute heart failure syndromes: an essential target of evaluation and treatment.

Patients with acute heart failure syndromes (AHFS) typically present with signs and symptoms of systemic and pulmonary congestion at admission. However, elevated left ventricular (LV) filling pressures (hemodynamic congestion) may be present days or weeks before systemic and pulmonary congestion develop, resulting in hospital admission. This "hemodynamic congestion," with or without clinical co...

Drugs acting on the heart: heart failure and coronary insufficiency

Heart failure (HF) and coronary insufficiency are common amongst surgical and critical care patients. Both are chronic conditions interrupted by acute episodes. HF activates neurohormonal mechanisms that worsen renal and cardiac function. Acute heart failure (AHF) commonly presents with dyspnoea as a consequence of systolic and / or diastolic dysfunction. Goals of treatment are symptom relief, ...

Diastolic dysfunction precedes hypoxia-induced mortality in dystrophic mice

Duchenne muscular dystrophy (DMD) is a progressive striated muscle disease that is characterized by skeletal muscle weakness with progressive respiratory and cardiac failure. Together respiratory and cardiac disease account for the majority of mortality in the DMD patient population. However, little is known regarding the effects of respiratory dysfunction on the dystrophic heart. The studies d...

Vein of Galen Aneurysmal Malformation and High-output Cardiac Failure in a Newborn

Background: Vein of Galen aneurysm (VGA) is the most common form of symptomatic cerebrovascular malformation in neonates. It develops in a fetus in the first trimester of pregnancy due to unknown reasons, but it is likely to have a genetic etiology. The prognosis of VGA is usually poor, particularly in newborns with heart failure due to high-flow intracerebral shunt. Surgery and endovascular em...

Efficacy of Oxycodone for Dyspnea in End-stage Heart Failure with Renal Insufficiency

A 67-year-old man with dilated cardiomyopathy and renal insufficiency was admitted to our hospital with dyspnea secondary to end-stage heart failure. We introduced oxycodone for medically refractory dyspnea instead of morphine because of the patient's renal insufficiency. After the administration of oxycodone, his dyspnea was alleviated without any adverse opioid effects, such as respiratory de...
